Is In Vitro Fertilization (IVF) the solution against miscarriages?
When a couple that a woman conceives naturally and miscarries should proceed to IVF?
The couple does not have to proceed to any therapeutic approach like IVF before the determination of the reason(s) that lead to palindrome pregnancies. So, the first thing that has to be done is the investigation of the causes of palindrome(s).
Recall that for miscarriages there is no “it happened”.
The reasons concerning the endometrium, such as infections of the female tract, thrombophilia and some immunological reasons, should be strictly corrected first and the couple should try normally if there is sufficient reproductive time.
In these cases, it makes sense to attempt IVF only if the causes of palindromes are corrected and the woman’s reproductive balance is marginal.
Conversely, if the scraping material after palindrome(s) is attributed to a chromosomal abnormality or if a member of the couple has a deviation from chromosome integrity, follow-up with IVF and preimplantation testing is recommended and required.
The above causes can be said to clearly point towards physical or assisted effort.
However, couples should be aware that there is another relatively common reason associated with miscarriage that is unfortunately not controllable. When in the first cell of the embryo and up to the immediately following divisions the DNA damage originating from the spermatozoon is not adequately corrected, then the prospects for the continuation of the pregnancy are not good even though the analysis of the chromosomes of the embryo can give a normal result.
Usually in these cases, the quality of the spermogram and/or sperm DNA deviates from normal and improvement should be sought before the next attempt and with great rigour. We remind you of something we say often. IVF does not overcome sperm problems in most cases.
Despite the above, and because of the couple’s impatience and because many gynaecologists feel they have nothing else to offer, many couples are pushed into assisted efforts without knowing exactly how IVF will help. In these cases the results are usually modest and unfortunately many first trimester palindromes result.
So it takes effort and patience to address sperm quality problems.
Also in cases where time passes and if the sperm count has not improved much, do not attempt intense stimulation with high egg production. A large crop of eggs should not be tied up with an unpredictable quality of sperm from a single ejaculation. And I generally recommend that when the sperm is of moderate quality, preimplantation testing should be done.
